Description

YSSN is currently seeking the services of a vendor who can provide a customized solution to improve their information management, data reporting and process management capabilities. In doing so, YSSN is looking to develop an integrated view of the data residing in the current databases and provide a more cohesive and higher quality client experience with a centralized view of client information and other relevant information. Through this solution, YSSN is also looking to increase its operational efficiencies through process automation and process management on a number of administrative activities.

Proposal submissions shall meet the following requirements:

A signed copy of the Proposal Submission Form, Declaration of Disclosure Form, and Conflict of Interest and Statement of Full Disclosure Form.
Proposals are limited to 15 pages, excluding, schedule, Proposal Submission Form, Declaration of Disclosure Form, Conflict of Interest and Statement of Full Disclosure Form, addendums, and appendices.
The RFP number "RFP-2020-05" shall appear in e-mail subject line. It is the Proponent's sole responsibility to ensure their submission is received in line with the specified expectation in the RFP.
Pricing Proposal is to be submitted as a separate file in the same e-mail
No company brochures are to be submitted.
Resumes may be referred to in an appendix.

Respondents shall include a detailed breakdown of the proposed total cost, including both one-time costs and ongoing costs, for the required services and solutions outlined.
Any additional costs, not considered as a requirement, shall also be listed separately in the Pricing Proposal.
