24.06.05 - Nurse call and Wandering System

The Horizon program is a 28-bed tertiary-level program that supports older adults living with neurocognitive disorders and/or mental health challenges. With the assistance of a specialized interprofessional team, the patients are supported by engaging in individualized care plans, including customized pharmacological and non-pharmacological interventions, as well and therapeutic and recreational activities. The patient population also consists of wandering patients with late-stage dementia, who are unaware of their environment and require constant monitoring. They are at risk to themselves and others.
The Horizon program does not have an effective means for patients to call for assistance and patient monitoring, leading to heightened risks of patient-to-patient and patient-to-staff violence, increased seclusion and restraint usage, and inadequate monitoring safety.
