1. The complete supply and installation of all labour, material, equipment and services to complete the Concrete Unit Paving Scope of Work. All work will be carried out in accordance with the tender documents, drawings, addendums, and specifications.
2. The Subcontractor to schedule a site review to examine current conditions and determine extent of required work.
3. This Subcontract includes for the supply and installation of all new concrete pavers as per SD2.2, Option No. 2.
4. Supply and Installation of Bedding and Joint Sand specified are included in this scope of work. Subcontractor to include approximately 1" of bedding sand.
5. The Subcontractor is responsible for all deliveries, unloading, hoisting, and installation of all materials.
6. When hoisting/lifting equipment is on site, include one hour free of charge daily for use by the Construction Manager.
7. Demolition, Concrete Stairs/Planters and Handrails are not included in this tender.
8. Replace the terms ‘engineer’ and ‘consultant’ with ‘construction manager’ in the specification sections.
9. Subcontractor to cooperate with testing and inspection companies to verify the installations.
10. Existing services will be located and paid for by the Construction Manager. The Subcontractor is responsible and shall pay for all repairs resulting from demolition / interruption of existing utility lines caused by Subcontractors work. In case of any interrupted or destroyed service line the Subcontractor shall arrange immediately for repair and inform the Contractor.
11. The subcontractor is responsible to coordinate with all other parties to ensure that the contract is completed in a timely and efficient manner.
12. Provide flagman to control and direct all associated equipment/vehicles required to complete the concrete unit paving scope of work.
13. The term “Contractor” or “Subcontractor” refers to the bidder herein.
14. Any and all requests for information should be forwarded to EllisDon in writing.
15. The Contractor is to complete Subguard Forms noted in Appendix 'C' of this tender package and qualify for Subguard Insurance in excess of the tendered amount. Subguard Insurance will be required prior to a contract award.
16. The Contractor is to provide Liability Insurance coverage as indicated with Schedule ‘I’, Insurance Policy prior to starting the work. The Contractor will be responsible for the deductible amount of $10,000 for the Builder’s Risk Insurance.
17. Submit monthly progress billings for approval no later than the 25th of the month for the entire month’s progress complete with a current WSIB Certificate and Statutory Declaration. Each progress billing must also include a breakdown of material and labour. Payment applications will be presented by the 5th of each month to the Owner by the Construction Manager for payment. The Owner shall make payment to the Construction Manager within 30 days of receipt of the payment application. The Construction Manager will make payment to the contractor within 10 days thereafter.
18. Trade Contracts are subject to 10% holdback. Following formal issuance of Certificate of Substantial Performance of the Project the Construction Manager is to make application for the release of holdback and submit the invoice to the Owner. Payment terms will be as noted in paragraph 5 above, except payment term will be 45 days.
19. All work is to be carried out in accordance with EllisDon's Site Specific Safety Policy available for viewing at the EllisDon Site Office. The Schedule ‘C’ (Safety Policy) is to be executed, signed, and returned to EllisDon prior to the contractor starting work on site.
20. EllisDon reserves the right to modify the schedule as necessary to ensure that the overall project deadline is met. Shift premiums shall be the responsibility of the subcontractor to ensure that the schedule is met.
21. The Contractor is responsible to coordinate with all other parties to ensure that the contract is completed in a timely and efficient manner. The contractor will work continuously until such time that their scope is deemed complete.
22. The Contractor is responsible for own survey and layout. Initial bench marks and major grid lines may be provided by the Construction Manager.
23. Arrange all delivery routes and storage areas with the EllisDon Site Superintendent prior to the start of work. Provide a flagman for deliveries of material and equipment to direct traffic and pedestrians as required. When working outside of fenced areas, provide enough flagmen to keep pedestrians and workers safe.
24. The Subcontractor is hereby instructed not to entertain any direct instructions from Owner or Consultants. All instructions are to be issued through the Construction Manager. If the Sub-Contractor reacts to any direct instructions other than those from the Construction Manager this is done at the Sub-Contractor's own risk and expense entirely.
25. Quality Control Requirements
- Attend Quality Control coordination meetings as required.
- Comply with Quality Control requirements as per plans and specifications.
- Provide Quality Control documentation as requested by the Construction Manager on a Monthly Basis.
- A fixed amount of $10,000 will be identified on progress billings for the submission and acceptance of Quality Control Documentation. This amount will be released at the sole discretion of the Construction Manager.
26. Site Access and Deliveries
- All related cartage, storage, hoisting, distribution costs are included.
- Includes all scaffolding, equipment lifts and other work platforms and supports as required by work of this subcontractor
- Access for the Subcontractor's deliveries and work will be coordinated with the Construction Manager’s Superintendent.
- Sub-Contractor is responsible for the repairs to any damage caused in transit or hoisting.
- Supply all necessary labour, material and equipment required to hoist all material to required areas. Supply and installation and maintenance of temporary protection to protect material deliveries to site is the responsibility of this subcontractor.
- Bidders shall review the jobsite and be fully aware of the proximity to adjacent activities and services. The subcontractor will take this into consideration during the planning and will submit procedures for the offloading, hoisting and installation for this scope of work for review by the Construction Manager prior to commencing work.
27. All escalation in material prices is included in this contract.
28. Subcontractor has included for phasing and multiple mobilizations that may be required to complete the Work.
