This request is for an innovative Electronic Health Record (EHR) system that can meet the business needs and functions, as a universally consistent tool, for the variety of clinical services provided by the in-scope agencies defined in the RFP.
Ideally, the solution will be cloud-based and can be accessed by all industry standard devices including a native app which can enable staff who work in the community to access the EHR from mobile technologies (Smartphone / Tablet).
The solution must include the capability to manage all existing data and reporting elements of the requestors current Information Systems and Client Records, including but not limited to: client/patient demographics, referral information, consents, letters of understanding, intake records, history, safety plans, service plans, assessments, contact notes, clinical/service/program notes, case reviews, incidents, medication history, discharge summaries, transfer summaries and group information for non-registered participants and contain a method to migrate the applicable data.
A key requirement is the ability to support an integrated Measurement-Based Care (MBC) service delivery model. This involves the capacity for clients (children, youth, and families) to complete validated clinical measurement tools within the platform and have discrete data scored and reported for clinician use in treatment planning.
