Pre-placement Medical Assessments

Objectives
This RFP is being issued by the Occupational Health and Safety Unit (OHS) of the TPS for the acquisition of a professional medical consulting service with the expertise and capabilities to provide numerous, comprehensive consistent physical examinations of applicants for

Police Officer positions
Specific civilian positions

The vendor will receive a copy of the duties and requirements of each position and an assessment will be made based on the essential competencies of the position.
The pre-placement medical assessments must

include vision and audiometric testing of Police Officer candidates, be performed in accordance with the provincial guidelines noted above,
and identify any pre-existing medical conditions (or treatment thereof) which could compromise the health and safety of a police constable or civilian member, his/her co-workers or the public, when the police constable or civilian member is performing critical physically demanding duties.

The Service intends to contract with one vendor to provide the services.

Mandatory Requirements

Copies of most up to date professional certifications/registrations as a physician, registered nurse, ophthalmologist/optometrist, audiologist etc. for proposed resources
Copies of memberships in good standing with respective professional bodies, e.g. College of Physicians of Ontario, etc.
Minimum 5 years’ experience in providing pre-placement medical assessments.
Ability to provide service to up to 30 applicants per week for one or two weeks, with a one week turn-around time frame. This situation may occur 1-3 times per year.
The selected vendor must be able to facilitate full testing capabilities by January 1, 2018.

Period of Performance
The term of this service is as follows:

Initial Period – January 1, 2018 to December 31, 2020
With the option to extend for two additional one year periods at the sole discretion of the TPS.

Submission Requirements
A two-envelope request for proposal (RFP) process will be utilized for this submission.

Proposal submission in Envelope One (technical response) will be sealed and contain one (1) original copy signed by an authorized officer of the company and three (3) additional copies of their proposal, which will include one digital/electronic PDF (searchable) copy on a USB flash drive (price excluded).
Proposal submission in Envelope Two (pricing response) will be sealed and will include an all-inclusive price associated with this RFP.

The original copies are to be clearly marked with the word ‘Original’ to be delivered to Purchasing Services, 2nd Floor, 40 College Street, Toronto, Ontario M5G 2J3, prior to 1:00:00 P.M. LOCAL TIME on September 22, 2017.
