Two (2) original hard copies of the Proposal should be submitted on 8 ½ inch by 11 inch paper, including any Addendums that may have been issued under Section 2. The submission must be clearly identified as “Request for Proposal – Employee Group Benefits Program”. A digital copy (pdf) of the Proposal shall be submitted on a flash drive as part of the submission with calculations and numbers in an excel-like format.
