Invitation to Proponents
This Request for Proposals ("RFP") is an invitation to prospective Proponents to submit Proposals for the provision of General Lab Supplies, as further described in Part 2 - The Deliverables (the “Deliverables"). This RFP is issued by the Ontario Agency for Health Protection and Promotion (“Public Health Ontario") (www.publichealthontario.ca).

Objective & Background
The objective of this RFP is to execute a contract with one legal entity to deliver specified amounts of General Lab Supplies to five (5) PHO laboratory sites (London, Orillia, Kingston and two (2) in Toronto) on a monthly delivery schedule as determined by PHO.

General Lab Supplies consist of the following three (3) categories:

Bags/Tapes/Liners
Test Tubes / Vials/Caps/Closures
Containers/Glassware

Timetable
The following is the schedule for this RFP:

Issue Date of RFP October 23, 2017
Proponent's Deadline for Questions November 2, 2017 @ 11:00 a.m.
Deadline for Issuing Addenda November 9, 2017
Proposal Submission Deadline November 28, 2017 @ 11:00 a.m. (Toronto time)

Period for which proposals are irrevocable after proposal submission Deadline 120 days

Contract Period
The term of the agreement is to be for a period of two (2) years. It is anticipated the agreement will be executed on or around January, 2018.

In order to be considered, proposals in response to this RFP must be received by the Ontario Agency for Health Protection and Promotion, no later than 11:00:00 am, Toronto time, November 21, 2017.

Proponents must obtain tender package through MERX.
