Description

Provincial Self-Management Health Care Professionals Training.
This RFB is an invitation from the Central East HCCSS on behalf of the 14 Self-Management Programs across Ontario seeking qualified vendors to provide health care professional training and support that is grounded in effective evidence-based approaches and techniques.
The health care professional training objectives for the 14 Self-Management programs are to ensure that health care professionals are trained in the principles and skills of self-management in order to provide effective support to individuals during healthcare interactions; to support information sharing and development of best practices in self-management and to build capacity across all regions using primarily a train-the-trainer model.
Scope of Work
Prior to April 1st, the 14 geographies have navigated several organizational changes, including the merger between Community Care Access Centres (CCACs) and LHINs in 2017, alignment and transition with Ontario Health (OH) in 2019, and the decoupling of Ontario Health and formation of HCCSS in July, 2021.
The Self-Management Program Central East is the only Self-Management program located within Home and Community Care Support Services across the province.
