Description

This Request for Proposal (RFP) is an invitation by the University of Ottawa to General Contractors or Electrical Contractors acting as General Contractors to perform the replacement of the football field (Matt Anthony Field) lighting at the Minto Sports Complex located at 801 King Edward Ave., Ottawa, ON.

MANDATORY PROPOSAL REQUIREMENTS

Completed Appendix A – Proposal Cover
Completed and signed Appendix B – Proposal Declaration Form
Completed and signed Appendix C – No Conviction of Fraud Declaration
Completed and signed Appendix D – Conflict of Interest and Disclosure of Information Declaration Form
WSIB - Provide a copy of Workplace Safety and Insurance Board (WSIB) Coverage or Letter of Exemption
Insurance Certificate - Provide proof of Insurance up to $5M CGL/Umbrella; Automobile Liability: $1M
Bid Bond - Provide Bid Bond of not less than 10% of the Bid Price inclusive of HST in favour of the Owner
Consent of Surety - Provide a Consent of Surety for Performance (50%) and Labour and Materials (50%) payment Bond
