The proponent should submit the following information in Envelope #1:
                              i.        Appendix B - Submission Form
                             ii.        Appendix D - Reference Form
                            iii.        Bid Submission Requirements as requested in Appendix E – RFP Particulars.
Proponents shall not include any cost information in the body of the administrative and technical proposal or it will be rejected. Do not include the Appendix C – Rate Bid Form(s), or any price information in this portion of your proposal.
