Mandatory Submission Requirements Include:
Completed Status in Relation to Manitoba Form (Form D1)
Completed Business Information and Experience Form (Form D2)
Completed and signed Form of Offer (Form D3)
Completed Price Form (Form D4)
Completed Contractor's Certification Form - Legislative and Safety Requirements of Manitoba Government Wide Contract Policy (Form D5)
Proposed High Level Schedule for the Project (Form D6) along with Gantt Chart

Mandatory Experience Requirements Include:
The Bidder must provide details in Form D2, Section A. or attach additional information to bid response that demonstrates the following:
a) Site supervisor assigned to the project has a minimum of five (5) years of window installation experience; and
b) Workers assigned to the project have a minimum of three (3) years of window installation experience.
