Project Name:

Orléans Health Hub Project (“OHH” or the “Project”)

Description of Project:

The OHH is bringing services closer to the home. The OHH is designed to support the needs of the Orléans community and surrounding areas in the East, and will be built on the principles of good governance and operating cost efficiency. The OHH is the first of its kind in Ontario, and is expected to drive community-based health care thinking across the Province.
The OHH will serve two target populations, who reside in Orléans and surrounding communities:

persons who have multiple and interrelated health conditions, are frequent users of the health system, and require an inter-professional and coordinated plan of care (including linkages between primary, secondary and community based services) to address their needs
persons who wish to access services at the OHH as it is more convenient, accessible and closer to home than local hospitals/other service providers

Partner organizations will transfer existing services and/or bring expertise to the OHH to enable the delivery of new model(s) of care that wrap around the client and address their full range of health and community support needs.
