Additional Bidding Instructions

1. The following items must be enclosed in the sealed envelope referred to in RFS section 7.2:

a. Price Quote and Submission Form. Completed and signed RFS Exhibit A - Price Quote and Submission Form.

c. Contract. Completed RFS Exhibit B – Contract Form. The legal name of the Supplier must be filled in on the first and signature pages of the Contract where indicated and the Supplier must sign on the signature page. The Contract should be completed in duplicate and all pages should be included.
