Additional Bidding Instructions

The proponent must submit their proposal in an electronic copy of their proposal via Merx with two separate attachments (files) containing the Proponent’s proposal labelled in the following manner:
a.         Envelope #1 Qualitative proposal NUMBER and insert proponent’s name which and Attachment #1 must include have the following mandatory documents:
i.      Appendix B – Submission form;
ii.     Appendix D Section C – Mandatory submission requirements;
iii.    Appendix D Section F – Rated criteria – do not put financial pricing information in this document. (one file per product submitted)
iv.    Appendix E- Reference consent form;

b.         Envelope #2 Financial proposal NUMBER and insert proponent’s name and Attachment #2 must have which must include the following documents:
i. Appendix C – Financial pricing sheet – (PDF) (one file per product submitted)

Do not include Pricing in the Envelope 1.
Pricing submitted incorrectly may be disqualified.
