Description

This Request for Proposal (RFP) is an invitation by the University to qualified Vendors (Proponents) to submit Proposals for fire extinguisher maintenance and inspection services as outlined in Appendix H (Service Requirements and Desired Outcomes) for a term of three (3) years with the option to extend the contract for one (1) additional term of two (2) years at mutual consent (total of five (5) years).

• Appendix D – Conflict of Interest and Disclosure of Information Declaration Form
• Appendix E – Evidence of Insurability
• Copy of Workplace Safety and Insurance Board coverage (WSIB)
AND
Attachment #2 Technical Rated Criteria
The following forms, information and attachments are to be included in Attachment #2 – Technical Rated Criteria:
• Section 1 - Annex I – Company Profile Form Template (2 pages)
• Section 2 - Annex II – Company Experience and Reference Verification Form Template (2 pages per project example)
• Section 3 – Annex III – Quality Assurance, Auditing and Quality Control Form Template - (2 pages)
NO PRICING INFORMATION MAY BE INCLUDED IN ENVELOPE #1. IF PRICING INFORMATION IS INCLUDED IN ENVELOPE #1, PROPONENT WILL BE DISQUALIFIED.
