Description

This Request for Supplier Qualification (RFSQ) is an invitation to prospective qualified Proponents to present prequalification submissions so that the University may establish four (4) VOR Arrangements for the following categories of Architectural Services which are further defined in Appendix F – Detailed Scope of Services and Expectations:
A. Interior Fit-ups
B. Complex Lab and Research Spaces
C. Light Retrofits, Upgrades, Rehabilitation
D. Exterior Works and Facades
Proponents may apply to more than one Category, but to do so, must complete certain parts of the Technical Rated Criteria for each Category for which they wish to be considered.
Refer to Article 2.2.2, Sections A, B, C and D. For clarity, Proponents are to tailor their responses as it pertains to the specific category they are applying to.
The five (5) highest scoring Proponents with regards to the evaluation criteria, will be Selected Proponent for each category, however the University reserves the right to increase or decrease the number of Selected Proponents based on the overall scoring of the Prequalification Submissions.
The University may add Shortlisted Proponents at a later date to potentially supplement the VOR Roster(s) at the University’s discretion.

As per Article 2.2.2 of this RFSQ, the following mandatory forms, information and attachments must be completed and submitted in Attachment #1 – Mandatory Requirements:

• M1 - Appendix A – Prequalification Submission Cover
• M2 - Appendix B – Prequalification Submission Declaration Form
• M3 - Appendix C – No Conviction of Fraud Declaration Form
• M4 - Appendix D – Conflict of Interest, Ethical Conduct and Disclosure of Information Declaration Form
• M5 – Appendix H – Corporate Overview Schedule
• M6 – copy of Certificate of Practice (issued by OAA)
• M7 - Copy of Workplace Safety and Insurance Board coverage (WSIB) (or provincial equivalent)
• M8 – Evidence of Insurability

Attachment #2 - Technical Rated Criteria
As per Article 2.2.3, the following forms, information, and attachment are to be included in Attachment #2 - Technical Rated Criteria:

• Part A – Annex I – Company Profile (Proponents need only submit a response to Part A once regardless of the number of categories being applied to)
• Part B - Annex II - Proposed Team, and Annex III - Project Experience & References (Proponents must respond to Part B for each Category they are applying to)
• Part C – Annex IV – Company Experience and References (Proponents must respond to Part C for each Category they are applying to)
• Part D – Annex V - Methodology (Proponents need only submit a response to Part D once regardless of the number of categories being applied to)

NO FINANCIAL INFORMATION MAY BE INCLUDED IN ENVELOPE #1. IF PRICING INFORMATION IS INCLUDED IN ENVELOPE #1, PROPONENT WILL BE DISQUALIFIED.
