HM_RFP_2017-18-0033 - Furniture & Equipment Consultant Services for Orléans Family Health Hub (OFHH)

Through this procurement process, Hôpital Montfort (Montfort), as the Project Authority and in collaborationwith the Partner Organizations for the OFHH Project, , is seeking the Services of a qualified Furniture and Equipment Consultant team of Personnel to support the requirements as described in the Statement of Work, in Appendix B.

Montfort is seeking to negotiate one (1) Contract (the “Form of Agreement”, as set out in Appendix A,
presents a sample form of Contract) as a result of this request for proposals (“RFP”) with a qualified
Supplier. The initial term of the Contract will be for a period of up to 36 months from the date of execution of the Contract. At Montfort’s exclusive option, the term may be extended by up to two additional 12 month periods.

This RFP is issued to invite you to submit a Proposal to provide Furniture and Equipment Consultant
Services (the “Services”), as described in this RFP, to and on behalf of Montfort, and on behalf and in
collaboration with the Partner Organizations for the OFHH. Proposals shall be in accordance with all of the requirements set forth herein
