Description

Grand River Hospital's Request for Quotation (“RFQ”) is an invitation to prospective bidders to submit quotations for the provision of stand-alone Infant CPAP unit/s for the hospital’s Childbirth program.

The goal of this RFQ is to establish a new infant CPAP machine standard for the next three years to support infant care at GRH. Vendors are required to review and submit responses to Schedule 1 – Evaluation Matrix and Schedule 2 Evaluation Matrix B (Financial Submission and complete other tasks noted on this document).

The Grand River Hospital Childbirth Program would like to acquire minimum of four (4) four standalone infant CPAP unit/s that meets the mandatory, clinical/technical, Biomedical and infection control requirements set out by the stakeholders. Warranty, service/support and training are also included in the evaluation criteria. Associated costs are evaluated to ensure the best overall value to the hospital. Additional units may be purchased contingent on funding available to the hospital. Therefore, bidders are encouraged to specify discounted pricing and value options available for each CPAP unit.
