Description

Central Regional Health Authority (Central Health) is administering this RFP on behalf of Eastern Health for CellaVision DM1200. This will be in accordance with the specifications and requirements set out in this RFP, as well as the Public Procurement Act.

The term of the agreement will be for a contract period of Two (2) years with an option to extend for an additional one (1) years.

The successful respondent(s) will be required to allow any or all of the Health Organizations (Eastern Regional Health Authority, Western Regional Health Authority, Labrador-Grenfell Regional Health Authority, Central Regional Health Authority, and NL Centre for Health Information) to avail of the goods and services outlined in the Tender.

The scope of this Tender is focused on the unique requirements of Eastern Health Due to this, each Health Organization will negotiate a new contract under a separate set of terms and pricing.

Additional Bidding Instructions

Bidders must complete and submit the Bid Sheet. Type or legibly print the information required on the Bid Sheet. All questions or areas on the Bid Sheet must be answered, even if it is only to indicate that the referenced item is not available; blank items will be assumed to be unavailable and may result in rejection of the Bid. Where yes/no questions are asked and the space is left blank by the Bidder, the assumption will be that the answer to the question is no.

All prices shall be provided in Canadian funds and shall include all applicable customs duties, tariffs, overhead, profit, permits, licenses, labor, carriage, insurance, and warranties, and further shall not be subject to adjustment for fluctuation in foreign exchange rates. All prices shall be quoted exclusive of applicable taxes. Taxes if applicable, should be stated separately.

The Proponent should provide the total cost of ownership as part of their bid sheet, including all anticipated costs related to the deliverables as identified in the Open Call documents, as well as other costs that may include but not be limited to transition costs; cost of installation; hourly rates and estimated hours required to perform the work; certification/testing costs; user training and education costs; discounts associated with volume increases; etc.

The participating Health Organization(s) reserve the right to order a different quantity of products than what is listed in the tender.
