Bidders are to submit:

1. 1. A signed copy of the Tender Form – Stipulated Price document; Tender Price Table; and all other required documentation as provided in the Specifications documents attached in Merx. Vendors must acknowledge each of the addenda relative to this tender that were posted to the web site have been read and understood by completing the required section on the tender form – stipulated price contract. Failure to complete the required documentation may result in the bid submission being deemed non-compliant.

All other required information where applicable and as specified in the document, may include but not limited to, bid bond, performance bond, material bond, COR certification, Proof of insurance, Proof of Registration and Good Standing with Worker’s Compensation Commission, and product literature.
