This Request for Proposals (this "RFP") is issued by Ontario Health as an invitation to prospective proponents to submit proposals for a solution to facilitate the collection, use and disclosure of patient-reported measures (i.e., patient reported experience and/or outcome measures) subject to a Proof of Technology process, as further described in Part 2 – Services. Ontario Health is looking for a sophisticated solution that can scale and spread our vision that focuses on capturing the patient journey within the healthcare system.
The solution will support Ontario Health’s strategic objective to transform care with the person at the centre. This is consistent with the Quadruple Aim to continue building a health care system that will improve the patient and provider experience, provider better, more connected care, and capture better value for health care dollars.
