Proponents shall submit eight (8) hard copies and one (1) electronic PDF (on USB stick or
CD) of the assessable criteria and a copy of the ‘Scope or Work’ in a sealed package marked with
the RFP title and number with the full legal name and return address of the
proponent and with the Submission Deadline. Proponents must initial the bottom of each page
in the ‘Scope of Work’ in the space provided.

Proponents shall submit one (1) hard copy of their pricing submitted on the ‘Rate Bid Form’
(provided in Appendix C) in an independently sealed envelope.

Do not submit or insert the ‘Rate Bid Form’ (or the monetary value of the bid) as part of the
assessable criteria in either the hardcopy or electronic format. A hardcopy format of the
‘Rate Bid Form’ is to be submitted only in an independently sealed envelope.
In the event of a conflict or inconsistency between the hard copy and the electronic copy of the
proposal, the hard copy of the proposal shall prevail.
